Cantex Continuing Care Earns a McKnight’s Excellence in Technology Gold Award for Using NexusConnexions to improve Care Transitions

Cantex Continuing Care Network, based in Texas, has won a Gold award at the 2018 McKnight’s Excellence in Technology Awards. The honor, in the Transitions category, recognizes its use of NexusConnexions software from Nexus Health Resources. The news was released from Middletown, New York, on October 5, 2018.

McKnight’s Long-Term Care News named Cantex among a small group of winners. The network had worked with Nexus Health Resources to roll out a new transitional care model across its facilities. Since then, according to the release, long-term patient outcomes have improved and avoidable hospital readmissions have fallen.

Lara Cline, director of care coordination at Cantex, said the software has made a big difference for its facilities. It builds a personal agenda for each patient and supports outreach and contact after discharge. As a result, she said, Cantex has cut readmissions after discharge by a significant margin. The tool also lets Cantex show referral sources exactly which staff interventions led to fewer readmissions.

NexusConnexions helps hospitals and skilled nursing facilities avoid needless costs and penalties. It does this by addressing the needs of patients who now live outside direct medical care. The patent-pending system assembles an agenda for every patient contact on its own. It then suggests dialogue and sets condition-specific goals, so staff can use proven tactics to improve outcomes and care plan adherence.

The McKnight’s program honors organizations that use technology to improve eldercare. In 2018 it was widened to cover Skilled Care and Senior Living as well.

Cantex operates in central, southern and eastern Texas. It offers short-term transitional care, rehabilitation therapy, chronic care, home health and hospice services, while its caregivers follow a philosophy called Committed to Excellence.

Nexus Health Resources provides transitional care software, services and patient engagement for acute and post-acute providers trying to cut readmissions and total medical spending. Besides NexusConnexions, it runs the Nexus Health Call Center and Nexus Health Academy, which offers condition-specific health education for patients and caregivers recovering at home.
