With the recent change in health care reimbursement, Dr. Feldman knew a key area of concern would be the critical period immediately after a patient is discharged from the hospital. “We basically wanted to empower the workforce, the transitional care coordinators, to improve their health population,” Dr. Feldman was quoted in the article.
