HUNTINGTON BEACH, Calif. and CARLSBAD, Calif., Nov. 16, 2017 –Landmark Health and its subsidiaries (“Landmark”), specializing in home-based medical care, and Ceresti Health, a company utilizing digital solutions to enhance personalized patient engagement, today announced the successful completion of patient enrollment in a controlled study of the Ceresti PREVENT program in a high-need Medicare Advantage population. Clinical and other outcomes from qualified Landmark patients enrolled in the Ceresti care coaching, education and support program will be evaluated against a control population.
